Insert Forms - Create New Items

Insert forms collect new data and automatically create items in your monday.com boards. Typical uses: lead generation, support tickets, project requests, event registrations.

Overview

Purpose: Add new items to your monday.com boards

How it works: External users submit data through the form β†’ New items are automatically created in your board

Security Options

By default, forms are open and accessible to everyone. All security mechanisms are optional.

Password Protection (Optional)

For insert forms, you can optionally enable a static password that you configure once for the form. This password is the same for all submissions.

Additional security options

  • Google reCAPTCHA and/or Cloudflare Turnstile: Optional captcha before submit (details)
  • Form restrictions: Limit access by geographic location (allowed countries)
  • Time limits: Start and end dates/times for form availability
  • Response limits: Maximum number of submissions

After you publish an insert form, use Test link in the FormFlow builder to open the published form safely. The test URL reflects the latest published version (not unsaved draft edits).

Prefill via URL (optional)

Published insert forms can accept query parameters on the public URL to suggest initial field values. See Default values and prefill. Because the link is public, do not put secrets in the query string. Use password protection when needed.

Setup Process

To create your first insert form:

  1. Go to your board and create a new form
  2. Choose β€œInsert Form” as your form type
  3. Map form fields to board columns
  4. Save your form configuration
  5. Optional: enable password protection, captcha, or other security options
  6. Optional: apply theming and styling to match your brand
  7. Publish the form to make it live and start collecting data

Common Use Cases

Lead Generation Form

Fields: Name, email, company, phone, lead source, interest level
Settings: Single submission per user, custom thank you message
Use Case: Collect qualified leads from your website or marketing campaigns

Support Ticket Form

Fields: Name, email, issue type, priority, description, attachments
Settings: Multiple submissions allowed
Use Case: Allow customers to submit support requests without accessing your monday.com board

Event Registration Form

Fields: Name, email, company, dietary restrictions, session preferences
Settings: Single submission, response limits, time limits
Use Case: Manage event registrations with automatic item creation in your board
