Build a Job Application Form in monday.com
Published on September 18, 2026
Resumes land in an inbox, a shared drive, or a stack of LinkedIn messages, and by the time a hiring manager wants to compare candidates, half the applications are scattered across tools nobody else can search. A recruiting board fixes the tracking problem. It does not fix the intake problem unless applicants can get their information onto that board without an account.
FormFlow builds the intake side: a public form that turns a submission into a board item, and a second, internal form that lets interviewers add notes and move a candidate through your pipeline without ever giving them board access. The two forms serve different people, so they need different settings.
Quick answer
- Use an insert form for the public-facing application — each submission creates a new candidate item on your recruiting board.
- Use an update form for the internal review step, with the candidate’s own answers set to read-only and only the interview fields left editable.
- Map file uploads for resumes and cover letters, and use a Dropdown column for the position so one form covers every open role.
- Install FormFlow and build the candidate application form first.
Why a Job Application Needs Two Forms
A public application form is built to be filled in once, by someone who is not part of your organization. An interview process is the opposite: the same item gets touched repeatedly, by multiple people, over days or weeks, and none of them should be able to edit what the candidate originally submitted.
Trying to do both with one form means either exposing the candidate’s raw submission to changes it should not have, or giving interviewers board access just to leave a note — which also hands them the whole recruiting board, not just the one candidate they’re reviewing. Splitting the workflow into an insert form for intake and an update form for review keeps each side scoped to what it actually needs to do.
monday.com’s own recruiting templates, including its job application form template , point at the same two-sided need: an intake form feeding a board, and a separate tracking view for the hiring team. FormFlow’s contribution is the second half — a way to let interviewers work the board without opening it to them.
Setting Up the Recruiting Board
Before building either form, set up the board columns both forms will map to.
| Column Type | Field | Used by |
|---|---|---|
| Text | Candidate Name | Application form |
| Candidate Email | Application form | |
| Phone | Candidate Phone | Application form |
| Dropdown | Position Applied For | Application form |
| Dropdown | Source (LinkedIn, Indeed, Referral, Other) | Application form |
| File | Resume | Application form |
| File | Cover Letter | Application form (optional) |
| Long Text | Candidate Notes | Application form (optional) |
| Status | Pipeline Stage | Internal review form |
| Long Text | Interviewer Notes | Internal review form |
| People | Assigned Interviewer | Internal review form |
| Link | Review Form Link | Generated by automation |
Keep the columns the candidate fills in separate from the columns your team fills in later. That split is what makes the read-only setup in the review form possible.
Building the Candidate Application Form
Step 1: Add FormFlow to Your Board
- Open your recruiting board.
- Click + to add a new view.
- Search for FormFlow and select it.
- Choose Insert Form as the form type, so every submission creates a new candidate item.
Step 2: Map the Candidate Fields
Map each field type to the matching column:
- Text for Candidate Name
- Email for Candidate Email
- Phone for Candidate Phone
- Dropdown for Position Applied For — list every open role so one link covers all of them
- Dropdown for Source, so you can see which channel is producing applicants
- File for Resume and Cover Letter — FormFlow accepts files up to 100 MB each, with up to 30 files per submission, so resumes and portfolio attachments both fit comfortably
Step 3: Add a Consent or Instructions Block
Use a Rich Text field near the top of the form to explain what happens after submission (expected response time, what documents are required) and to include any consent language your legal team requires before collecting personal data. An Image Embed or Video Embed field next to it can show a team photo or a short welcome clip from the hiring manager, instead of relying on text alone to set the tone.
Step 4: Apply Branding and Publish
Use FormFlow’s theming options so the application looks like part of your company site rather than a generic form, then publish it and share the link on your careers page or job boards.
Step 5: Add Spam Protection
A public application form on a careers page draws bot submissions. Turn on reCAPTCHA or Cloudflare Turnstile in form settings before you publish, especially if the link is posted somewhere widely indexed like a job board.
Building the Internal Review Form
The review form is what turns a stack of applications into a working pipeline. It is an update form, so it edits the candidate item that the application form already created — no duplicate items, and the interviewer’s notes land on the same row the candidate applied through.
Step 1: Create the Update Form
Add a second FormFlow view to the same board and choose Update Form. Map:
- Pipeline Stage (Status) — Applied, Phone Screen, Interview, Offer, Rejected
- Interviewer Notes (Long Text) — free-form notes from each round
- Assigned Interviewer (People) — who is currently handling the candidate
Step 2: Show the Candidate’s Answers as Read-Only
Add the candidate-facing fields (Name, Email, Position, Resume, Source) to the same form so interviewers have full context, then mark each one read-only. Interviewers see exactly what the candidate submitted but cannot accidentally overwrite it while leaving notes.
Step 3: Narrow the Form by Stage With Visibility Rules
Not every interviewer needs every field. Use visibility rules to show or hide sections based on Pipeline Stage — for example, only show an “Offer Details” section once Pipeline Stage is “Offer,” so a first-round interviewer’s form stays short.
Step 4: Generate a Link Per Candidate
Update forms need a unique link per item before anyone can open them. Set up a monday.com automation:
- Trigger: When an item is created (i.e., when a new application comes in)
- Action: Generate link for form → save to the Review Form Link column
Interviewers open that link, see the candidate’s read-only details, and update the stage and their notes — no board access required. See automating form links for the general automation pattern this uses.
Step 5: Restrict Access if Needed
If interview notes contain anything sensitive, add a per-item password to the review form and generate it alongside the link, or set allowed countries if your hiring team only reviews from specific locations.
A Complete Candidate Flow
- A candidate finds the job posting and applies through the public application form (insert form).
- FormFlow creates a new item on the recruiting board with their name, resume, and position applied for.
- An automation generates a per-item review form link and stores it on the item.
- The recruiter shares that link with the assigned interviewer, who opens it, reads the candidate’s read-only details, and sets Pipeline Stage to “Phone Screen” with their notes.
- As the candidate advances, each interviewer reopens the same link, sees the accumulated notes from prior rounds, and adds their own.
- When a decision is made, Pipeline Stage moves to “Offer” or “Rejected,” and a monday.com automation can notify the recruiter or trigger next steps.
Nobody outside your team edits the application form’s original answers, and nobody needs a monday.com seat to leave interview feedback.
Frequently Asked Questions
Can one application form cover multiple open positions?
Yes. Use a Dropdown field for “Position Applied For” listing every open role, so you only maintain one public form instead of one per posting.
Do interviewers need a monday.com account to leave notes?
No. The internal review form is a FormFlow update form with a unique link per candidate. Interviewers open the link and submit notes without board access or a monday.com login.
Can a candidate accidentally overwrite the interviewer’s notes?
No. The application form and the review form are separate FormFlow views. Candidates only ever see the public application form; they have no link to the review form at all.
Do I need the Enterprise plan for this setup?
No. Insert forms, update forms, visibility rules, and read-only fields are not gated by your monday.com plan tier in FormFlow.
